The ID Card Re-Issuance module allows employees to raise a request for re-issuing their access/ID card in cases where the existing card is no longer usable.
Who Can Use This Module
- Employees – to submit a re-issue request
- Managers / HR / Admin – to review, approve, and process requests
When Can a Re-Issue Request Be Raised
An employee can request a re-issue of the ID card under the following scenarios:
- Lost ID Card
- Damaged ID Card
- Print Faded / Unreadable
- Other Reasons

Request Flow
- The employee initiates an ID Card Re-Issuance Request.
- The employee fills in the required basic details to submit the request.
- The request is routed through a multi-level approval workflow, which may include:
- Reporting Manager
- Fourth Floor Authority (if the request is raised by a Fourth Floor employee)
- HR
- Admin
- Once the request receives final approval, a new ID card is issued to the employee.
